Looking for an autism questionnaire for yourself or your child? Find recognised screening tools, understand their limits and prepare for a professional autism assessment.
Screening is a starting point, not a diagnosis.
A questionnaire cannot confirm or rule out autism. NeuroVillage provides information and links to official tools; we do not provide a clinical assessment or diagnostic result.
Explore your child’s experiences, one everyday situation at a time.
These original NeuroVillage prompts cover experiences described in public autism guidance. They have not been collected in a parent study or clinically validated. This is not M-CHAT or an autism assessment, and it cannot calculate autism likelihood.
Think about your child’s developmental stage and experiences across settings. Choose what you have noticed; skip anything that does not fit. These experiences can have many explanations. Repetitive movement can be enjoyable or regulating, and communication can include speech, signs, gestures or assistive tools.
No names or free-text personal details are requested. The summary is generated on this page, with no answer-saving or submission code. Keep any additional examples privately.

Your next step does not depend on a score
If development or daily life concerns you, talk to a GP or health visitor, even if you mark few or none of these observations. Bring examples, their impact, what helps and your child’s strengths. A qualified team considers a much wider history.
If your child has lost skills they previously used, seek medical advice promptly. For difficulty swallowing or coughing/choking during meals, ask for an urgent GP appointment or contact NHS 111.
Start with a GP to ask about the local referral route. For a child, a school or health visitor may also be involved. Availability and processes differ across the UK. You can ask about support while waiting; you do not need an online questionnaire result before raising a concern.
Can an online autism questionnaire diagnose me or my child?
No. Screening can help organise a conversation, but diagnosis requires a comprehensive assessment by qualified professionals. High scores can have different explanations, and low scores do not exclude autism.
Are the official questionnaires free?
The tool owners make questionnaires available under their own terms. Follow the links above for current access and instructions. NeuroVillage charges nothing to use this guide.
Why do the standard questionnaires open on another website?
We link to the original owners so you can use their current, unaltered questionnaires and instructions. Republishing and scoring these tools on a commercial website requires appropriate permission. Our parent experience questionnaire is separate and is not a replacement for a standardised tool.
What if the questionnaire result does not match my experience?
Discuss your experiences and concerns with a GP or qualified assessor. You can ask for an explanation, appropriate support and information about assessment routes regardless of a screening score.
